Since the update to tvOS18 the lag on DIRECTV Stream has been awful. Live and recorded shows lag, but commercials play fine. I’ve restarted, reset and tried WiFi and Ethernet without resolution. Help me Apple.

Same issues here. Lag on syncing with DIRECTV. Had shadowing issues with live sports etc. unwatchable.
I fixed it by turning off the Match Content feature in the appletv settings. All good now.
Update: turning off Match in System and restarting seems to have helped. I was already set to 4kHDR.
Turning off match and setting to 4K HDR worked for me. It seems backwards, turning off match to get match, but it works.
Unfortunately my TV only has SDR so that option won’t work for me. :-(
Same issue as variously described above. Didn’t always occur on all channels, seemed like some more than others. I turned off match settings and kept the 4k hdr setting on. Seems to be working.
I had the same issue only with directv on a Roku and switching display type on the Roku to 4k 60fps automatic vs manual fixed the issue.
