how do i recover my camera icon to my home page?
You have not disclosed which iPhone you are using. You have not disclosed which iOS version you are using. I will therefore provide a general resolution for the issue that you are facing (Camera app does not appear on the home screen).
Open Settings ➔ General ➔ Reset ➔ Reset Home Screen Layout. This action will return the Camera app to your home screen and rearrange the apps to their default positions.

while I still might have your attention...I have a 27" Mac with the latest OS.......I can initiate a FaceTime call on the Mac but it will no receive a call however it will on my iPhone.....how do I toggle this back to the Mac for receiving?

Ensure that your Mac and your iPhone are using the same Apple ID and then Double Check the following settings-
On your iPhone
Open Settings ➔ Phone ➔ Calls on other devices : Ensure that this is set to ON and your Mac is listed below that. Find the listed Mac and ensure that the switch for the Mac is set to ON.
On your Mac
Open FaceTime ➔ Preferences ➔ You can be reached for FaceTime at : Ensure that your Phone number is checked. Calls from iPhone is checked.

thank you Axel....I'm using the latest IOS and new iPhone SE....my camera is now found but my contact icon is now missing...any thoughts?....on second search it was found...thank you again

All boxes were checked on both devices and my daughter faceted me as a test and it only came through on the iPhone......

Does the FaceTime on your mac have a phone number checked or an email address checked or both? Same question for your iPhone.

Yes, on the MAC a phone number is checked as well as my 3 e-mail addresses.
On the Iphone the "allow calls on other devices" is checked and calls are allowed on my iMac

All. of these features were working until two days ago when I updated my OS to the most current OS...

How about signing out of FaceTime on your Mac and then signing back in again? Try that.

I tried that and when my wife attempted to FaceTime the response said "failed"
