Here's the way it works:

Phone unlocked/watch unlocked - notifications sound on the phone.

Phone locked/watch unlocked - notifications sound on the watch

Phone locked/watch locked - notifications sound on the phone

You can turn off wrist detection in the Watch app in which case the watch will almost always be locked and you'll generally get notifications on your phone. But, you'll need to unlock the watch to do pretty much anything but look at the time and you won't be able to use Apple Pay.

You can also go to the Watch app>Notifications and scroll down to the section for installed third party apps. I just noticed the other day that, if I turned Spark mail to "off" there, I heard the notifications on my phone. I don't know if that was a glitch or not.

Hi do not disturb is not enabled. All sound notifications are enabled. I’m setting up an alarm system and when I activate it I only get sound with my alert when my phone is unlocked. It’s frustrating. I’ve seen a bunch of questions on here regarding this. DND is not on on my phone, the only thing checked is the “silence when iPhone is locked”.

If DND is turned off, none of the settings on that page affect anything. There is some other reason you're not getting the alert.

Do you have the mute switch engaged? It's the slider above the volume buttons?

Check if you have unknown callers silenced. Settings>Phone>Silence Unknown Callers and Settings>Messages>Unknown & Spam>Filter Unknown Senders.

The mute button is not ok. The phone rings for calls. Just notification sounds don’t happen. Notifications are enabled for the app as well. I’ve seen other ppl post the same comment so I don’t buy that DND being off means that “silence while locked” isn’t enabled. Too many ppl are experiencing the same thing. I believe it’s a feature of an Apple update that needs to be turned off.

You can believe what you like but those people who post the same problem generally figure out what the real cause is. Just for kicks, I have repeatedly tested this. I have mine set for "Always" under DND. When DND is off, I get notifications as expected. When it's on, I don't get them. And, I've tested it, repeatedly, with "While Phone is locked". Same thing. It always works as advertised. So, either the cause of your problem is something else or there is something wrong with your specific phone.

Standard trouble shooting steps would include resetting your settings in Settings>General>Reset>Reset All Settings, restoring your phone from a back up and testing, restoring to to factory state and testing.
