I have AppleTV3 and I did get the fields to enter the 6 digit number.
have you made sure Apple TV is updated to latest?
as a work around in your Apple ID page disable 2 factor authentication. That way you will not get prompted for this.
Yes. Make sure it is up to date (7.4), then you will see the TV icon and can access that in order to view Apple TV+
I assume you have the Apple TV 3, right?
Have you tried watching the Apple TV+? They have a trial for 7 days so it will also not cost you anything.
