In the Health app, tap Search (or the sidebar icon on iPad), then Medications. Scroll down to Your Medications, tap the one you want to delete, then scroll to Options and tap Delete Medication.

You can also archive the medication instead, if you want to keep the medication details and log history for your records. The steps are the same as above, just swipe left on the medication instead of tapping it.
