I would like to use an Apple watch for fall detection and SOS around my home. I have wifi, and iPad and a landline but no cell phone.

Technically, the Watch is an iPhone accessory. You need an iPhone to set up an Watch.
If another family member has an iPhone, they could set your watch up "for a family member."
