For the stolen iPhone, no, you cannot:

  • remove something not associated with an Apple Account.
  • remotely add a device to an Apple Account.
  • remotely track by serial number or IMEI.
  • remotely add a device to Find My.

The usual procedure for a stolen iPhone follows, but unfortunately little of this is applicable as the iPhone has already been separated from your Apple Account:

Your carrier can potentially block a stolen device by its IMEI.

As for the replacement iPhone, reset it, restore your iCloud or local backup (if available), and that iPhone will be logged into iCloud automatically as you log the iPhone into your (recovered) Apple Account as part of the initial setup.
