Cannot Add Vaccination Card to Wallet

I am trying to add my latest vaccine info to my iPhone Wallet app.

When I scan the QR code from the government document, my Health app vaccine info gets updated, but I get a message:

"Unable to Add to Wallet

The vaccination record has been added to Immunisations in Health. The vaccination card could not be added to Wallet at this time."

Why? I have never had this issue before.

I have always canned the QR on the PDF and the record is added to both the Health and Wallet app

I did notice that iOS reads the QR Source as the Government of Ontario Canada, but the Status says Not Verified.
