Siri, Voice Control activate unexpectedly

I have an iPhone 15 with iOS 18.4. It is a 2023 model with 256 GB storage. I alternate between using Apple earbuds and Airpods Pro 2. First, Siri started interrupting Apple Podcasts and video apps. Sometimes, Siri would make the podcast go 2x. I turned off all of Siri as best I could and reset the iPhone. Then, Voice Control started interrupting Apple Podcasts and video apps. Also made podcasts sometimes go 2X faster. I turned off all of Voice Control as best I could and reset the iPhone again. Now, Voice Control continues to interrupt. It even interrupts calls. It happens when the iPhone is in the pocket of my hoodie and when the iPhone is sitting flat on a table. Due to AT&T convincing me long ago to make my business phone lines into mobile numbers, this is my primary business phone. So, I need this to stop! What is your best diagnosis and fix?
