one of the basic features of a phone today is to have alarms, I wake up with my phone alarm everyday. I have my regular alarm set at 7am every weekday, however once in a while I need to wake up earlier. For example I need to take my dad to a Dr appointment on October 1st so that specific day I need to wake up at 5am. So how do you add an alarm(not a reminder) for October 1st at 5am?? Or is this one of those things Apple can’t do…

You might look at an app like Calarm which sets alarms based on calendar events.
If you're ambitious, you might be able to set up a Shortcut to do what you want. But that's not really my area of expertise.

I would hardly call it a "basic" function. I suspect there is not nearly as much call for it as you think.
You can not specify a date for an alarm. You would have to set the 5:00 AM alarm the day before you need it.
Set a reminder for yourself for September 30'th to remind you to set an alarm for 5:00.
