Setting up a profile to disallow the speakers would block the speakers from being used. Bit, this is coming from the app. So, create a profile, and see if you can add it while on a call. Yes. Both audio input and audio output would be blocked. And to do that would require disallowing all of airplay with the execution of certain devices.

As an Alternative:

Make sure your Mac and iPhone are running macOSVentura and iOS 16, or later. Then sign them both into the same Apple ID account. Your iPhone should now appear as an option in the microphone dropdown menu for whatever app you're using.
