Haptics volume being automatically turned to near zero without me doing anything

Haptics volume in settings is being turned to almost zero without me opening the settings app or doing anything. I usually have it set permanently around 50% and have had no issues for the past few years. My phone has been on silent and do not disturb since pretty much the founding of the country, but after it started I tried taking my phone off DND and silent, and it still happens, and neither of these should affect it in the first place. Iphone 15 pro max IOS18.3.1 up to date as of 2/28/2025. I have haptics volume change with buttons setting disabled. I have attention aware features disabled.

I only realize when I sleep through an alarm and realize I never heard it, or mid day I'll feel the phone buzz in the pocket but no noise, or miss a call from someone and I catch their text later that they were unable to reach me. Every time I have to manually change the haptics volume in settings from 0 to 50%, and it works fine for another day and then randomly changes on its own.

This issue has been going on for months and its extremely dangerous and worrisome. I cannot afford to miss appointments and exams which I have nearly done so many times because of this issue. It must be resolved now.

If I need to provide any additional relevant information let me know. Any information appreciated. Apple please fix this.
