Why hasn't the refund appeared on my Apple Card?

Still not exactly clear, but I can make a guess that you requested a refund through Apple for the Apple TV+ service and got an approval notice. All refunds would return to the card that was used for the payment and you can review the page here for the amount of time it may take. For a credit card that could take up to 30 days, but usually you will see it sooner than that.
