“you have not completed the entire form”

Hi, my debit card that I used for subscriptions in my iPhone, was stolen so I blocked it. I got a new card but when I try to added it in “payment & delivery” the message in red says “you have not completed the entire form”. Due I cannot add new payment method I have no more iCloud storage and other subscriptions in general. Does someone have any solution?
