If you get someone an apple watch with gps and cellular, if they dont want to pay the cellular monthy can they use it as just gps.

Yes. They don't have to setup the cellular function. See the 'Set up cellular on Apple Watch' section in:

"You can also set up cellular later from the Apple Watch app..."
