Can I play music from Spotify on my Bluetooth headphones from the watch? I want to leave my phone at home when I go for a run. I just got the Apple Watch 5 with cellular and I set it up correctly.

Spotify does not support the Apple Watch, beyond being a remote control for their iPhone app. As Spotify is not an Apple product, you would have talk to Spotify about their future plans for the Apple Watch.
