Photos on the Mac (as léonie suggests) or a specialized app like QuickTime should preserve the resolution. I didn't see the choice on QuickTime, but VLC for Mac (it's free!) has a "Snapshot" feature-- but I didn't find it on the iPhone version. You could VLC on a PC, I'm sure…
I would probably open the video it in Quicktime and run the video until you got to the point where you have the area where you want to capture a screen and do a screenshot. Shift+Command+4
Thanks Al. Would using another app (QT) provide better resolution than just screen grabbing the shot in Photos? I was hoping to get something better than “just” a screen grab quality.
Short answer is no you cannot extract a still photo from video in Photos app.

Always much better quality to do it on a Mac and Quicktime than on your phone, as detailed by Léonie.
I do it a different way, though. 1. Play the video in Quicktime. 2. At the frame you want a still of, stop playback and hit Command-C (copy). 3. Open Preview and hit Command-N. 4. Save the still image.
