Fix by:

  1. Turning iCloud Keychain OFF → restart → ON
  2. Removing CloudKit Keychain metadata
  3. Signing out → into iCloud to refresh security tokens
  4. Resetting KeychainCloudInformation
  5. Resetting NVRAM
  6. Temporarily disabling Advanced Data Protection
  7. Rebuilding the local keychain (guaranteed fix)

Your passwords are stored safely in iCloud — this is only a sync issues on the Mac.
