How can I improve Apple Watch's ability to read through tattoos?

You'll find this is a problem with any tracker that uses light to read heart rate. Some are slightly better, some worse. But all will have problems. People with extremely dark skin may also have problems.

Use your other arm. Place it on an area that isn't tattooed.

There are substances in the ink that block the optical technology used in the Apple Watch and many other fitness tracking products.

There is nothing you can do to alter that.

You could consider have the tattoos in Apple Watch area of your wrist removed.

You would not like how much power the Apple Watch would have to apply to get light through the tattoo in both directions so the Apple Watch could sense the vanes & arteries in your wrist. It would A) make for a burning of your skin, B) it would compound this by making the Apple Watch physically hot to expend that much energy, and C) it would drain the battery very quickly (it is a very small battery).
