Epic Games wanted to have their own App Store to distribute the game, so now it is only available in the EU.
What is the error that you’re getting when installing Fortnite? You might need to update your macOS to the latest version in order to install Fortnite
You can only install Fortnite using the Epic Games Store, that is only avaliavle in the EU

Funny that their state says:
Apple terminated our ability to develop Fortnite for Mac back in 2020 without explanation.
All the while Epic Games has made many public statements about the reason and their intention to have their own App Store. This also includes years of lawsuits where they fully understood what the reason was, but simply did not agree with it. They knew exactly what they were doing and is a little disingenuous to come across as a poor developer that Apple won't allow in the App Store without knowing what the reason is.
