Why does Health app reset medication log after already recording it taken?

I have medications set for 4:30 am and 5:30 am and many more later. I also track mealtimes as medications. Some days I log them taken earlier and other days later, but I have noticed on occasion that that the first medication resets to no logged time after already having set a time taken that morning.

I use Health to track when I take a medication and this is inconvenient on days when it resets and I cannot remember what time it was when I took that first pill.
