"Change mono audio and balance settings
You can also open the Apple Watch app on your iPhone, tap My Watch, tap Accessibility, then turn on Mono Audio and adjust the audio balance."

Issue resolved. On Apple Watch Settings —Accessibility there is a setting with a green line to balance sound between left and right. My green line was all the way to the right. Solved issue by adjusting green line to halfway (3 blocks in)
