why is my apple pen glitching when i write?

I am trying to take notes and my pen writes in almost patches and will create random lines. Overall it's very glitchy

Check to make sure the tip is securely screwed into the pencil. Also check to make sure the tip of the pencil is not worn where the tip meets the iPad screen.
