For any Apple Watch above the SE, once you choose the LTE & GPS, do you have to pay monthly still?

If you connect the cellular connection of the watch to your carrier then yes there will be a monthly charge from them.
If you decide to use the watch without the cellular connection as a GPS model then no there will be no charge as long as you don't connect it with your provider.
