The back glass is fused to the phone and it cannot be replaced. If you bought AppleCare+ when you bought the phone, Apple will replace the phone under that plan for $29. If you didn't buy AppleCare+ you'll have to pay for an Out of Warranty replacement, which will cost you $449.
