I know the passcode to my grandmothers phone but how do i unlock it if it is disabled?

You must follow the stews in If you forgot the passcode on your iPhone, or your iPhone is disabled, even if you remember the passcode.

Unless you previously backed the iPhone up, you can't. There's no longer any place on it you can put the passcode.
