My daughters iPad was set up using my info. Several months later I got an iPhone. They are synced because of my info. My daughter is 9, and I do need control over her iPad, however I need my phone to be MINE!! How can I separate them!

Log out of your account on her phone, then create a new Apple ID for her on her phone. Be sure to specify her correct age in the setup. Then set up Family Sharing with you as the organizer, and your daughter as a minor family member.
