I’ve set up the Dock Favourites in the Watch app on my iPhoneX but it doesn’t turn up when I push the doc button on my Apple Watch 4.
I'm left with the full list of apps.
Recents doesn't sync either.
Any ideas?

It sounds like your pressing the Digital Crown instead of the Side Button on the Apple Watch. Pressing the Crown will take you to either a list of apps or a grid of apps. Pressing the Side Button opens the dock.
